BellaLisse is dedicated to providing a first-class client experience with the highest level of functional and integrative health care. We view our relationship with you as collaborative and want you to play an active role in your care.
We value your experience and opinions and invite you to share them with us. We will make every effort to educate you about your treatment, and the specific recommendations we are making, at each step in the process. Please read the material below to familiarize yourself with our policies and procedures. Nature of Services Provided
To ensure your safety and provide the best treatment approach, consultations are always performed prior to vitamin infusions, injections, and face & body treatments.
Please note that BellaLisse does not function like a traditional doctor’s office. It is best to consider us as a consultant that provides specific clinical services, rather than as a primary care provider.
We do not have a receptionist on staff to answer the phone (although we do return voicemail messages during specific hours each day), and our web-based Electronic Health Record is the primary means of communication.
We do not handle emergencies and it may take us up to 72 hours to respond to your inquiry.
If you experience an emergency in connection with your treatment at BellaLisse, please call 911 or visit the nearest emergency room.
Clinic Etiquette
Arrival: Please arrive a few minutes prior to your scheduled appointment time. New clients may have forms to complete prior to your appointment. We understand that the unexpected can occur and it may be difficult to arrive on time for your appointment. If this happens please call and let us know you will be late. We will try to accommodate you; however, out of respect for other clients’ time, we cannot guarantee the full-service time if you are late. We will do what we can in the time allowed. If you are more than 15 minutes late you may be asked to reschedule.
Cell phones: to maintain the quiet & serene atmosphere of the clinic we ask that you place your cell phone on silent upon check-in.
Sickness: Please reschedule your appointment if you are sick or getting over a sickness. It is in the interest of everyone’s safety that we refuse treatment and ask you to reschedule if you show up for an appointment with noticeable or questionable symptoms.
Treatment Room Policy: Due to Covid-19 only the client receiving a service is allowed in the treatment room at the time of service. All other guests must either wait in the waiting area or outside the clinic. In the interest of safety for your children and in consideration of other guests’ privacy we ask that you not bring children to your appointment.
Please note: BellaLisse reserves the right to refuse service at any time, to anyone, for any reason. It is also at the clinic’s discretion to stop or refuse a service if the service provider feels unsafe, verbally or physically harassed, or mistreated by a client.
Billing Policies
PAYMENT POLICY
Payment is due when services are rendered. We do not send invoices to clients or insurance companies or coordinate with insurance companies on your behalf.
We accept cash and all major credit cards. Personal or travelers’ checks are not accepted.
CANCELLATION/REFUND POLICY
Your appointments are very important to BellaLisse and are reserved specifically for you. We realize that sometimes schedule adjustments need to happen and we kindly request at least a 24-hour notice for cancellation so that we may offer the appointment time to another client.
If you cancel or reschedule at least 24 hours prior to your appointment there is no charge.
If you cancel or reschedule less than 24 hours prior to your appointment you will be charged as follows:
- First occurrence: no fee
- Subsequent occurrences: 50 percent of the appointment fee
Appointments made within 24 hours that need to cancel should cancel within 4 hours of the appointment time or be subject to:
- First occurrence: no fee
- Subsequent occurrences: 50 percent of the appointment fee
No shows or same-day cancellations that are less than 4 hours of your appointment time:
- 50% of the service will be charged in order to reschedule.
Complimentary services: No shows or same-day cancellations that are less than 4 hours of your appointment time:
- $25 consultation fee to rebook
Treatments: All injectable and face & body treatment sales are final; refunds or credits cannot be offered once treatment is completed.
Laboratory tests: We will refund the cost of any laboratory tests not performed to your credit card on file. We cannot refund lab tests after the lab has processed your results.
Supplements & Products: Unopened, unused supplements and products in their original packaging are eligible for a full refund, minus any shipping cost, within 14 days of purchase. Opened purchases may be considered for BellaLisse credit only if they are more than one-half full for a partial in-store credit only. If any allergic reaction to the product occurs, the product must be returned immediately to receive full credit, as most reactions occur with 24 to 48 hours.
Insurance Responsibility and Claims Management
Bellalisse strongly recommends that all clients maintain health insurance coverage, but does not participate in insurance, health plans, or government payers, nor does it accept assignment from any other payer, including employers. The client is responsible for all charges and fees incurred for treatment or services rendered, regardless of any insurance coverage. Insurance reimbursements vary significantly as to health care services, and Bellalisse makes no representations as to what services may or may not be covered under any insurance or health plan, or by any government payer, such as Medicare or Medicaid.
